North Carolina has lost at least ten rural hospitals since 2005, according to a hospital-closures tracker kept by the University of North Carolina at Chapel Hill.
These are the small North Carolina cities that still combine a relaxed getaway with good medical access.
1. Hendersonville
Hendersonville centers on a walkable, orchard-country downtown, backed by a hospital now carrying a higher trauma-care tier.
Main Street runs several blocks of independent shops and restaurants, with Henderson County’s apple orchards and the Blue Ridge Parkway a short drive from downtown.
UNC Health Pardee anchors the city’s medical care as part of the University of North Carolina’s statewide hospital network.
The hospital recently earned Level III trauma center designation, adding faster on-site care for serious injuries across western North Carolina.
Not a small upgrade.
Pardee also runs a cancer center, and the hospital says it was the first in the Carolinas to earn International Organization for Standardization (ISO) 9001 certification for quality management.
2. Boone
Boone sits high enough in the Blue Ridge Mountains that summer visitors pack a jacket along with sunscreen.
The Blue Ridge Parkway runs along the edge of town, and Blowing Rock’s shops and mountain overlooks sit a short drive away.
A certified stroke center, Watauga Medical Center also holds a Gold Seal chest-pain certification from the Joint Commission.
Mountain hospitals rarely stack both.
The hospital’s cancer program also carries approval from the American College of Surgeons.
3. Brevard
Brevard calls itself the Land of Waterfalls, and the trailheads inside Pisgah National Forest back up the claim.
White squirrels, a documented local population, dart across downtown lawns while the Brevard Music Center runs its summer concert season nearby.
Full accreditation from the Joint Commission covers Transylvania Regional Hospital, which treats stroke patients around the clock.

4. Waynesville
Waynesville’s Main Street climbs a hill lined with galleries.
For four decades, the town hosted Folkmoot USA, an international folk festival that ran from 1984 to 2024.
Its board then retired the annual festival for year-round arts programming.
The Blue Ridge Parkway and the edge of the Great Smoky Mountains sit minutes from downtown.

In 2025, the state’s emergency medical services office verified UNC Health Blue Ridge as a Level III trauma center.
That took over a year.
The designation means the hospital can stabilize serious injuries on-site instead of sending every case toward Asheville or Charlotte first.
6. Pinehurst
Pinehurst was designed in 1895 by Frederick Law Olmsted, and the area’s tourism board still calls it the Home of American Golf.
The village hosted the U.S. Open four times since 1999, most recently in 2024, with a return already booked for 2029.
The Reid Heart Center and a dedicated cancer center both run out of FirstHealth Moore Regional Hospital’s 402 licensed beds.

Old Baldy, North Carolina’s oldest standing lighthouse, sits across the water and doubles as a small museum.
As a federally designated critical access hospital, Dosher Memorial still holds advanced certification for hip and knee replacement, including Mako robotic-assisted surgery.
Not bad at all.

Two Hospitals, Same Trauma Tier
What a Level III Trauma Center Guarantees
UNC Health Blue Ridge and UNC Health Pardee both carry North Carolina’s Level III trauma verification, a tier few travelers check before booking a trip.
The designation requires emergency physicians in the building around the clock, plus general surgeons and orthopedic surgery on call.
Many cases stop right there.
For the most complex cases, severe brain injuries or multi-system trauma, the hospital stabilizes patients on-site and transfers them to a Level I or Level II center.
